Immunizations and Communicable Disease

Whenever people are together in group settings, there is a potential for infectious disease to spread. To help protect your child, other students, and staff from vaccine-preventable diseases, the State of Idaho requires some immunizations before children can attend a public school. Pursuant to Idaho Code 39-4802, a parent/guardian may exempt their child from any or all immunizations required by the State of Idaho by submitting one of the following items to front office staff at your child's school:

  1. A medical exemption signed by a physician licensed by the Idaho State Board of Medicine. The exemption can be a note signed by your healthcare provider or you may fill out the Immunization Exemption Form linked below (not required).

  2. A statement signed by a parent/guardian stating their objection to immunizations for their child based on religious beliefs.
