All of our student athletes must be registered on Aktivate, formerly known as Register My Athlete. This is an online platform where you will set up an account, enter all of your student’s information and upload the required documents right into the same account. We are moving entirely away from all paper registration, nor will we be excepting any documents here at the school. We are attaching a how-to video for you to watch, along with written instructions on how to set up your account and complete the process. Our high schools and other middle school already use the same program. Thank you so much for working with us to properly register your student for athletics at Lakes Middle School!

Requirements/Forms
Aktivate Registration, ASB Fee ($25.00) and a Sports Physical Examination are REQUIRED for participation in all sports.
A student’s physical is good for ALL three years at Lakes Middle School.
REQUIREMENTS TO PARTICIPATE
PHYSICAL-Complete one during middle school years. It must be on file in Aktivate. Copies are available in the office.
ATHLETIC QUESTIONNAIRE/CONTRACT- Completed each school year on Aktivate.
INSURANCE - Athletes are required to sign a form acknowledging insurance coverage or purchase of school insurance. This information, including insurance company name AND POLICY NUMBER, must be included on the above mentioned contract form on Aktivate.
ASB STICKER - Athletes are REQUIRED TO PURCHASE an ASB Sticker for their Student Body Card. ASB cost is $15.00.
This helps cover the cost of officials and equipment. This can purchased in the front office or on Aktivate.
MEDICAL EMERGENCY AUTHORIZATION FORM - Athletes are required to fill out this form every year on Aktivate.
