REGISTER HERE - Our Online Athlete Registration process is now live. All athletes past and present will need to follow this link to register. Here’s a quick “getting started” tutorial.

Please plan for the process to take from 10-30 minutes. The sports physical is the only document that will need to be uploaded or turned in to the office. If you have turned in a sports physical already during your student’s time at Canfield, we will apply that to your online registration from the office.

Sports Physical  –  Must be uploaded through Register my Athlete or turned in to the office PRIOR to the first practice. This physical is good for all three years of middle school and can be completed by your family practitioner (form can be printed from Canfield Athletic Page if Dr. office does not have one) or urgent care providers.

Asb Activity Pass  – All athletes are required to purchase their student body card. The cost is $15. Please purchase this on Skyward (Information about efunds), at open house (new students), or at the office during regular office hours. If you purchase this after ASB cards are printed you will need to have your student come to the office for an "Activity" sticker.
