Health Conditions: Please report any health issue your child has that could impact safety and learning at school to the school nurse. You can contact the nurse directly by phone or e-mail. It is important you update your student's health registration form once a year so the school nurse is aware of any new or ongoing health concerns.
Life Threatening Health Conditions: If your child has an anaphylaxis allergy , asthma, diabetes, seizure disorder, heart disorder or other life threatening condition, please contact the school nurse and complete the appropriate forms on the Health Services Web site (2020-21 School Health forms) If your child needs any medical treatments at school please notify the school nurse.
Medications: If your child takes or needs to have any medication (prescription or over the counter) at school we need a medication authorization form signed by a licensed health care provider and parent for each medication. The medication should be brought to the office by an adult. Please see the link on our Health Services Web site (2020-21 School Health forms ) for the appropriate documentation that needs to be filled out.
Immunizations: Please provide updated immunization information to the school so that state requirements can be accurately recorded and your child is protected from vaccine preventable diseases. Please see the link for Idaho requirements and schedules
